Appendix D - Web Site Evaluation FormThe link below will open a PDF file. The file contains a form useful in evaluating a Web site based on the criteria outlined in the this report. The Survey can be filled out using the Acrobat Reader Software, and the results can be printed. Completed surveys cannot be saved, but the blank form may be downloaded and saved for later use. The form will automatically calculate a "score" if Acrobat Reader 4.0 (or higher) is used to complete it.
